ATTESTATION: By my signature, I attest that I am a currently serving as a law enforcement officer (municipal, county, state, or federal), a member of the U.S. military forces (active or reserve), a firefighter, or as an EMT/paramedic, or honorably retired from any of these services.  I understand and agree that any misrepresentation of my status constitutes fraud, and may subject me to severe criminal and civil penalties.
